Application Forms for Affected RatnaKalakars and Affected Units

Attached are two separate application forms :

1) School Fee Sahay Application Form 

The affected artisans will have to obtain the online “School Fee Assistance Application Form” from the link given above and submit it along with the supporting documents as per the checklist to the school where the applicant’s children are studying by “23/07/2025”. Application forms will not be accepted after 23/07/2025. Please note that.

અસરગ્રસ્ત રત્નકલાકારો ઉપરોકત આપેલ લિંકમાંથી ઓનલાઇન “ સ્કુલ ફી સહાય અરજી ફોર્મ ” મેળવી ચેકલિસ્ટ મુજબના આધાર- પુરાવા સાથે અરજદારના બાળકો જે શાળામાં અભ્યાસ કરતા હોય તે શાળામાં “તા.૨૩/૦૭/૨૦૨૫” સુધીમાં જમા કરાવવાના રહેશે. તા.૨૩/૦૭/૨૦૨૫ બાદ અરજી ફોર્મ સ્વીકારવામાં આવશે નહી. જેની નોંધ લેશો.

2) Vyaj Sahay Application Form

While the affected units will have to get the online “Vyaj Sahay Application Form” from the given link and submit it along with the supporting documents as per the checklist to the office of District Industries Center, Surat by “23/06/2025”. Application forms will not be accepted after 23/06/2025. Please take note of this.

જ્યારે અસરગ્રસ્ત એકમોએ આપેલ લિંકમાંથી ઓનલાઇન “ વ્યાજ સહાય અરજી ફોર્મ ” મેળવી ચેકલિસ્ટ મુજબના આધાર- પુરાવા સાથે જિલ્લા ઉદ્યોગ કેન્દ્ર, સુરતની કચેરીએ “ તા.૨૩/૦૬/૨૦૨૫ ” સુધીમાં જમા કરાવવાના રહેશે. તા.૨૩/૦૬/૨૦૨૫ બાદ અરજી ફોર્મ સ્વીકારવામાં આવશે નહી. જેની નોંધ લેશો.
